• BPC-157 is generally well-tolerated, but users report several side effects ranging from mild and transient to severe and persistent.  The most common issues include headaches, nausea, dizziness, fatigue, and injection site irritation, which typically resolve within 24–72 hours. 

  More serious or less common adverse reactions reported in anecdotal communities and limited clinical observations include:

  • Neurological and Mood Changes: Anxiety, depression, brain fog, insomnia, and severe mood swings. 

  • Cardiovascular Issues: Elevated blood pressure, heart palpitations, and increased resting heart rate.

  • Gastrointestinal Distress: Severe bloating, constipation, diarrhea, and gut motility issues, particularly in those with pre-existing conditions like SIBO or hypothyroidism. 

  • Allergic Reactions: Rashes, hives, blistering, and swelling.

  Important Considerations:

• Acute/Loading Dose: 1,000 mcg total per day (500 mcg of each peptide), split into two doses (AM/PM) for the first 4–6 weeks to maintain stable blood levels due to BPC-157’s short half-life. 

• TB-500 Specifics: In separate-vial protocols, TB-500 is often dosed at 2–2.5 mg twice weekly during the loading phase, tapering to once weekly for maintenance.  In pre-mixed blends, the TB-500 is delivered proportionally with the daily BPC-157 dose. 

Reconstitution and Measurement

For a standard 20 mg vial (10 mg BPC-157 + 10 mg TB-500) reconstituted with 2 mL of bacteriostatic water:

• Concentration: 10 mg/mL total (5 mg/mL of each peptide). 

• 500 mcg Dose: Draw 5 units (0.05 mL) on a U-100 insulin syringe. 

• 1,000 mcg Dose: Draw 10 units (0.1 mL) on a U-100 insulin syringe.
